We use "DataPacks" to collect essential information from our potential clients. By downloading, printing and filling out these forms before your first appointment, you take an important first step toward being pro-actively involved in your case and, further, you help reduce your legal costs.

If you would prefer, we would be happy to sit down with you and fill out these forms with you, but you must understand that you will have to pay an attorney or a paralegal to do this for you. Wouldn't you rather do it yourself?

Before downloading, viewing and printing any of the following forms, you will have to have Adobe Acrobat installed on your computer. This is a very widely used "multi-platform" program for distributing forms and other data. The software is available for free from Adobe at:
